NHS England – Important Request to Confirm Your Details

NHS England is carrying out checks to ensure that GP practice patient records are accurate and up to date.

As part of this process, some patients may receive a communication from NHS England asking them to confirm their personal details, including their current home address.

How might I be contacted?

You may receive a request by:

· Letter

· Email

· Text message

· Notification through the NHS App

Your communication may include a QR code which you can scan to access the online service and confirm your details.

What do I need to do?

If you receive a request from NHS England, please respond within the timescale stated in the communication.

If your details are correct, simply confirm them.

If you have moved address, please provide your new details as requested.

Why is this important?

It is important that the NHS has accurate information about where patients live. Up-to-date details help ensure that you receive important healthcare information, including:

· Screening invitations

· Vaccination information

· Test results and other healthcare communications

· Information about routine NHS services

NHS England uses patient-list validation processes to identify records where details may need to be checked. Patients are given opportunities to confirm or update their information before any removal from a GP practice list is considered.

Please don't ignore the request

If you do not respond, and NHS England and the practice are unable to establish that you are still resident at the address recorded or remain registered with the practice, this may ultimately result in you being removed from the practice patient list.

Closure Afternoons

It is essential that all staff attend regular training updates for the benefit of patient care.

The next Practice closure afternoon will be Wednesday 17th September 2026 the surgery is closed from 1pm until the following day.

All calls will automatically be redirected to NHS 111.
